Women taking isotretinoin should not become pregnant during and for one month after stopping isotretinoin therapy. Sexual abstinence or effective contraception is recommended during this period and may be prescribed by doctors in the US and EU. [2] Barrier methods alone (e.g., condoms) are not considered appropriate due to unacceptable failure rates of approximately 3%. It is generally recommended that women who become pregnant during isotretinoin treatment have an abortion. Isotretinoin is the only non-psychiatric drug on the FDA`s Top 10 Depression-Related Drugs list[41][45] and is also in the top 10 suicide attempts. [46] A black box warning of suicide, depression and psychosis has been on the packaging of isotretinoin in the United States since 2005. [45] In March 2018, the European Medicines Agency warned of a possible risk of neuropsychiatric disorders (such as depression, anxiety and mood swings) following the use of oral retinoids, including isotretinoin, although the limitations of the available data did not allow them to clearly determine whether this risk was due to the use of retinoids. [36] A revised isotretinoin pregnancy prevention program was implemented in 2005. The program consists of 3 parts: Information, Treatment and Prescription. Aggressive chemical dermabrasion and laser skin treatment should be avoided in patients treated with isotretinoin for a period of 5 to 6 months after discontinuation of treatment due to the risk of hypertrophic scarring in atypical areas and, less commonly, hyperpigmentation or post-inflammatory hypopigmentation in treated areas. Waxing should be avoided in patients taking isotretinoin for at least 6 months after treatment due to the risk of epidermal stripping. Isotretinoin has been associated with inflammatory bowel disease (including regional ileitis) in patients without a history of bowel disease. Patients with severe (haemorrhagic) diarrhoea should discontinue isotretinoin immediately.

Renal impairment does not significantly reduce the plasma clearance of isotretinoin or 4-oxo isotretinoin. The literature was reviewed in pubmed and psychinfo for research related to isotretinoin and neuropsychiatric side effects such as depression, suicidal thoughts, suicide, mania, anxiety, impulsivity, emotional lability, violence, aggression and psychosis. Several case series have shown that successful acne treatment with isotretinoin leads to improved quality of life and self-esteem, but studies show isolated cases of clinically significant depression and other neuropsychiatric events that, although uncommon, are persistent in the literature. Since the first cases of depression were reported to the U.S. Food and Drug Administration, numerous cases have been reported to regulatory agencies in the U.K., France, Ireland, Denmark, Australia, Canada and other countries, making isotretinoin one of the top five drugs in the world associated with depression and other neuropsychiatric side effects. Clinicians are advised to warn patients of the risks of neuropsychiatric side effects of isotretinoin that may result from the drug itself, and not just as a side effect of acne or youth. Isotretinoin has also been linked to psychosis. [23] Many of the side effects of isotretinoin mimic hypervitaminosis A, which has been linked to psychotic symptoms. [40] The dopaminergic hypothesis of schizophrenia and psychosis suggests that an increase in dopaminergic stimulation or sensitivity in the limbic system causes psychotic symptoms. [48] Cases of benign intracranial hypertension have been reported, some of which involved concomitant use of tetracyclines (see sections 4.3 and 4.5). Signs and symptoms of benign intracranial hypertension include headache, nausea and vomiting, blurred vision, and papillary edema.

Patients who develop benign intracranial hypertension should immediately discontinue isotretinoin. Renal impairment and renal impairment do not affect the pharmacokinetics of isotretinoin. Experiments completed in 1971 showed that the compound was probably ineffective in cancer, but surprisingly could be useful for treating acne. However, they also showed that the compound likely causes birth defects, so Roche dropped the product in light of thalidomide events. In 1979, the New England Journal of Medicine published an article demonstrating the effectiveness of the drug in treating cystic and conglobate acne in fourteen patients, thirteen of whom experienced complete elimination of their disease. [64] Roche has resumed work on the drug. In clinical trials, subjects were carefully screened to avoid women who were or might become pregnant. Roche`s new drug application for isotretinoin for the treatment of acne included data showing that the drug causes birth defects in rabbits.
